ТРОМБОЗЫ В ОНКОЛОГИИ. ЧАСТЬ 1

Аннотация

Наличие онкологического заболевания повышает риск развития артериального тромбоза преимущественно в коронарном и цереброваскулярном русле. Среди патогенетических механизмов данного осложнения рассматривают прямой токсический эффект на эндотелий у некоторых групп химиопрепаратов, прокоагуляционный эффект опухоли, угнетение эндогенного фибринолиза и повышение агрегации тромбоцитов. Важным представляется и активизация воспаления за счет нарастания синтеза медиаторов воспаления и адгезивных молекул, а также вазоспастический и прямой повреждающий эффекты лучевой терапии на эндотелий. У больных с ОКС, возникшем на фоне активного онкопроцесса, с одной стороны, имеется высокий риск тромботических осложнений, а с другой — ограничения в антитромбоцитарной терапии из-за возникающей тромбоцитопении, поэтому в принятии решения о возможности, составе и длительности антитромбоцитарной терапии у подобных больных, наряду с кардиологами, должны участвовать онкологи и химиотерапевты. Статья представлена в двух частях.
 В первой рассматриваются особенности состояния гемостаза при онкологическом заболевании, некоторые механизмы патогенеза развития артериальных и венозных тромбозов, а также особенности развития и лечения онкологических пациентов с развитием артериальных тромбозов. Во второй части представлен обзор литературы по профилактике и лечению венозных тромбоэмболических осложнений, ассоциированных с онкологическим заболеванием.
