ПРОФИЛАКТИКА ВЕНОЗНЫХ ТРОМБОЭМБОЛИЧЕСКИХ ОСЛОЖНЕНИЙ У ПАЦИЕНТОВ С АКТИВНЫМ ОНКОЛОГИЧЕСКИМ ЗАБОЛЕВАНИЕМ, ПОЛУЧАЮЩИХ МЕДИКАМЕНТОЗНУЮ ПРОТИВОРАКОВУЮ ХИМИОТЕРАПИЮ В АМБУЛАТОРНЫХ УСЛОВИЯХ. РОЛЬ АПИКСАБАНА

Аннотация

Онкологическое заболевание является одним из самых значимых факторов риска развития венозных тромбоэмболических осложнений (ВТЭО). Лечение уже подтвержденного ВТЭО у онкологического пациента всегда сопряжено с дополнительными трудностями, которые связаны с лекарственными взаимодействиями, что может повлиять на эффективность химиотерапии или эффективность и безопасность антикоагулянтной терапии. Профилактика ВТЭО у онкологических пациентов могла бы занять лидирующие позиции. Тем не менее до сих пор остается неясным, нужно ли проводить медикаментозную тромбопрофилактику пациентом с онкологическими заболеваниями, так как тромботические риски и риски геморрагических осложнений широко варьируются и зависят от типа онкологического заболевания, стадии процесса и проводимого противоракового лечения. Четко критерии тромбопрофилактики определены у госпитализированных пациентов и пациентов, которым проводится хирургическое лечение. Однако длительная терапия рака преимущественно проводится в амбулаторных условиях. Проведение химиотерапии увеличивает риск развития ВТЭО. Новая таргетная терапия, которая проводится самостоятельно или в комбинации с традиционной химиотерапией, не уменьшает риск развития ВТЭО. «Золотым стандартом» по лечению и профилактике ВТЭО у онкологических пациентов являются низкомолекулярные гепарины. При использовании НМГ, особенно в группах онкологических пациентов с высоким тромботическим риском, было показано достоверное снижение тромботических событий, но кровотечения остаются главным неблагоприятным влиянием, нивелирующим пользу от медикаментозной тромбопрофилактики. Еще одна попытка увеличить безопасность тромбопрофилактики – это использование прямых пероральных антикоагулянтов (ППОАК). В статье рассматриваются современные положения медикаментозной тромбопрофилактики у пациентов с онкологическими заболеваниями, а также возможности использования ППОАК, в частности апиксабана, для профилактики рак-ассоциированных ВТЭО.
